CHKS

The California Healthy Kids Survey (CHKS) is an anonymous, confidential survey of youth resiliency, protective factors, and risk behaviors. It is administered to students at grades six, seven, nine, and eleven. It enables schools and communities to collect and analyze data regarding local youth health risks and behaviors, school connectedness, protective factors, and school violence. Included in the elementary survey this year are 6-8 questions regarding tobacco use. The CHKS is part of a comprehensive data-driven decision-making process on improving school climate and the student learning environment for overall school improvements. 

 

The supplemental module chosen is Social Emotional Health and is a strength-based assessment of youth empathy, self-efficacy, self-awareness, persistence, emotional self-regulation and behavioral self-control. 

 

Please respond by February 9, 2024. You only need to submit once per student. 

 

6th grade Active Consent

Active Consent requires a parent or legal guardian be notified and written permission for the student to participate is needed.

 

7th, 9th, 11th  Passive Consent (Opt Out)

Passive Consent requires a parent or legal guardian be notified and given the opportunity to opt the student out of participating.
